My job makes better lives by promoting health and independence for older adults' in-patients with complex physical and mental health problems by delivering high quality physiotherapy assessments, care planning and interventions to achieve excellent outcomes for the people who use our services, and their carers.
Job Overview
To assess, plan, implement and evaluate the physiotherapy needs of people with learning disabilities who use our services.
To supervise and teach unqualified staff including students.
To foster a culture that encourages and enables individuals to have an active role in identifying, planning, and meeting their individual needs.
Main duties of the job
* To carry out comprehensive assessments, plan, implement and evaluate delivery of physiotherapy interventions as part of a holistic approach working in a variety of settings.
* To demonstrate specialist physiotherapy assessments such as mobility, falls, tone management - using a variety of modalities.
* To do full postural assessments and lead on the 24-hour postural management programmes. Additional internal and external training will be provided to ensure proficiency in this.
* To work as part of a multi-professional team to help holistically treat the service user.
* To actively involve service users and their carers (where required) in developing and agreeing their own care plan, to act as an advocate and ensure that there is proper regard for their dignity, choice, self-esteem, racial, cultural, sexual, and religious needs and preferences.
* To practice within the Positive Behaviour Support framework.
* To undertake comprehensive risk assessments and develop effective risk management plans.
* To work in partnership with acute and primary care services and other healthcare providers.
* To establish and maintain effective relationships with colleagues, other agencies, people who use services and their families.
