Community Healthcare Assistant - Toressa Community Nursing Team. Permanent, part-time (37 hours per week; evenings and weekends required). Band 3. Location: Toressa Community Nurses, Torpoint / Saltash. Salary: £24,937 - £26,598 per annum. Job ref: 201-25-673. Closing date: 23/09/2025 23:59.
We are an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health, and provide specialist support to people with dementia or a learning disability. Responsibilities
* Carry out assigned tasks providing direct care to patients and carers to support the registered nurse.
* Work as a member of the community nursing team within a defined geographical area.
* Deliver planned nursing care in patients’ homes, including care for end-of-life patients (e.g., wound care, venepuncture, catheter care, pressure area care, personal care).
* Travel independently daily to and from the base, patients’ homes, and other venues around the geographical area.
* Contribute to integrated ways of working to achieve best patient outcomes.
Qualifications and Skills
* Essential: NVQ Level 3 in care or equivalent experience; GCSE Maths & English or equivalent; evidence of experience in a care setting; trained in venepuncture or willingness to undertake training.
* Desirable: Additional relevant healthcare training.
* Essential: Good communication skills; ability to work without direct supervision and within time restraints; ability to stay calm in distressing situations; ability to cope under pressure.
Additional Information
* Travel to and from the base and patient locations is required; some locations may not be accessible by public transport.

* Working pattern: service operates 8am to 10pm with 7.5-hour shifts; home-based and community settings.
