Post Diagnostic Support Link Worker – South Perthshire
Alzheimer Scotland is looking for a Post Diagnostic Support Link Worker to work within the Community Mental Health Team at Crieff Hospital. The role is part of the National Team of PDS Link Workers, providing high‑quality, person‑centred support to people living with dementia and their families for a minimum of a year following diagnosis.
Contract: Temporary, part‑time, 21 hours per week (12‑month fixed‑term contract)
Location: Crieff Hospital, Kings Centre, King Street, Crieff, PH7 3HR
Responsibilities
* Support people with a recent diagnosis of dementia to live well in their community
* Provide high‑quality, person‑centred support for at least one year following diagnosis, in line with the Scottish Government’s minimum guarantee
* Use Alzheimer Scotland’s 5‑pillar model to help individuals understand their illness, come to terms with the diagnosis, and plan for future care and decision‑making
* Facilitate peer support, maintain links with community resources, and develop outcome‑focused plans to support future hopes, desires, and aspirations
* Collaborate closely with colleagues within Alzheimer Scotland, the NHS, and other stakeholders, and build and maintain relationships
* Use digital platforms for virtual meetings and online groups as part of a blended approach to support people
Qualifications and Skills
* Recognised professional qualification at level SVQ 4 or equivalent (or other relevant qualification)
* Good understanding of dementia and its impact on people and families
* Excellent communication skills, warm flexible approach, empathy, and relationship‑building ability
* Skills and knowledge at the Enhanced Level of the Promoting Excellence Framework (or evidence of progression towards completion)
* Full driving licence and access to a car
* Subject to criminal check through Disclosure Scotland
