Lookingfor a role where you will be working with some of the city's most vulnerablepeople. With the Frailty team your rolewill primarily be providing preventative assessments to support the team inpreventing progression of frailty and unnecessary admissions to hospital.Providing short term interventions that will support maintaining safety andindependence at home. You will also have the opportunity to work in other areasincluding expediting early discharges from hospital.
Youwill be working as part of an expanding Frailty HUB which has anticipatory,crisis and discharge arms. We integrate with other services, both primary andsecondary care, social services, CVS and YAS.

Main duties of the job
Tocomplete physiotherapy assessments to in the community preventing admission tohospital and providing community rehabilitation. This will include treatmentplanning, assessing risk, promoting, and enabling independent functioning.
Toprovide training, supervision and advice to therapy assistants, and othertherapists/ nurses in the team
Toparticipate in the planning, development, and evaluation of community therapyservice, holding responsibility for defined projects.
Tomaintain close working relationships with other health professionals /referrersto ensure efficient, effective, and appropriate delivery of the service.
Afull driving licence and access to a vehicle that can be used for work isessential.

To undertake the specialist Physiotherapy assessmentand management, of a designated group of patients, within the specialty ofpatients within the Community Frailty Team. This will involve working withindividuals, groups, carers and relevant others, providing specialistassessment and treatment in the home environment to prevent progression ofFrailty.
To determine clinical needand intervention indicated. To work as an autonomous practitioner; holdingresponsibility for own caseload of patients with complex physical,psychological and cognitive problems.
To provide support,specialist advice and training to more junior members of staff and students. Toact as a resource providing information and advice regarding patients withinthe Community Frailty Team.
To maintain records as anautonomous practitioner in accordance with legislation, professional andservice policy. To contribute to the collection of business-related informationto inform service development and priorities.
To work flexibly as a memberof the department responding to relevant service needs.
Provide support to othercolleagues when dealing with this group of patients.
To be responsible for adefined area of service working without direct supervision. Clinical work isnot routinely evaluated; however clinical work will be evaluated in line withPrimary Care and service objectives. Access and advice is available from seniorprofessional colleagues if required.
Initiate evidence-based audit and projects toevaluate and improve clinical practice. Initiate and evaluate user involvementand where appropriate make local service changes.
Development of Physiotherapy within The CommunityFrailty Team.
To contribute to appropriateclinical networks which may be at a local, regional or national level.
To provide professional,competent and efficient care which meets patients health and social needs andthat promotes a reduction in the number of inappropriate hospital admissions.
MainDuties and Responsibilities:
To undertake a comprehensive assessment of patients,including those with diverse or complex/ multi-pathology presentations.Specialist functional assessment of cognitive, perceptual, sensory and motorskills, which may include the use of standardised testing. Use advancedclinical reasoning skills and assessment techniques, to provide an accuratetreatment plan and to evaluate any intervention.
To formulate and deliver a specialist, individual, goal-orientatedtreatment programme based on sound knowledge of evidence-based practice andtreatment options using assessment, clinical reasoning skills and knowledge oftreatment e.g. treatment techniques and principles, equipment provision andother alternative options.
To maintain robust workingrelationships with the wider MDT including social care and voluntary sectorcolleagues.
To provide evidence basedassessment and patient centred treatment for Frail patients which are SMART andgoal focussed.
Take responsibility fororganising, triaging, and prioritising own caseload in line with service andpatient needs. Adapt and readjust plans as situations change or arise, ensuringtimely and effective interventions. Appropriately delegate duties to othermembers of staff to support safe, efficient, and high-quality service delivery
Motivate and encourage patients to participate intreatment programmes and where
necessaryalter treatment to maximise potential. To include families, carers and supportstaff
To use a range of verbal and non-verbalcommunication tools to communicate effectively
withpatients and relevant others. The caseload will include individuals who havedifficulties
inunderstanding or communicating.
