We are seeking an Advanced Clinical/Nurse Practitioner to join our friendly, forward-thinking team. The ideal candidate will have experience working within Primary or Secondary Care in an ACP role and will be able to work autonomously, seeing complex patients alongside the wider MDT.
Main duties of the job
To assist medical personnel in caring for practice patients by providing assessment, screening, treatment services, and health education advice. Assess, diagnose, plan, implement, and evaluate treatments and interventions for patients presenting with undifferentiated diagnoses or minor illnesses. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs, in accordance with evidence-based practice, national, and practice protocols, and within scope of practice. Undertake care for patients in their homes as required by the Partners. Be competent and confident in managing people with long-term health conditions. Diagnose and manage both acute and chronic conditions, integrating drug- and nondrug-based treatments into management plans. Prioritize health problems and intervene appropriately in complex, urgent, or emergency situations, including initiating effective emergency care.

Person Specification
Experience
* Minimum 2 years post-registration experience
* Management of long-term conditions
* Implementation and use of protocols and clinical guidelines
* Experience of audit
* Experience in triaging
* Participation in quality initiatives such as clinical benchmarking
* Experience within Primary Care
Qualifications
* Registered Nurse
* Advanced Clinical Practitioner level 7
