Overview
An experienced Learning Disability Nurse is needed to join the Adult Community Learning Disability Team in Fylde and Wyre. The Learning Disability Service delivers forward‑thinking, high‑quality care in partnership with other agencies, supporting adults with learning disabilities and autism, their families and the wider community.
Key responsibilities
* Provide clinical support to adults with learning disabilities, including those with complex needs such as challenging behaviour, physical and sensory impairments, mental health and forensic issues.
* Deliver care within a variety of community settings and work collaboratively with the integrated health team.
* Play an integral role in the delivery of pathways for these patients.
Qualifications & experience
* Registered nurse, with up‑to‑date knowledge of health policy for people with learning disabilities, including the Transforming Care agenda.
* Adaptability, strong communication skills and self‑motivation.
* Proven skills in working with people who have complex health needs and challenging behaviour.
* Interest or experience in one of the following areas: Positive Behavioural Support, mental health, forensic issues, or acute/primary care liaison.
