We are looking for a highly motivated individual to work in our Community Coordination Centre based at Milford hospital. We recognise that administrators are vital to the smooth operation of all our services. You will act as the first contact for all enquiries via telephone and email, log referrals on EMIS, our electronic clinical system and complete all general administration duties. The Adult Community Service division of the Royal Surrey Foundation Trust are work in partnership with local GP practices in Guildford and Waverley to develop and provide integrated services for the population living in the area. Health professionals in the community, including GPs, nurses, therapists and paramedics are working closely to provide a completely integrated service for patients not only providing great care, but improving patient experience too. You will join an established administration team based at Milford Community Hospital, working alongside our team of dedicated professionals including nurses, paramedics, therapists, and support workers. You will have a great support network which includes regular team meetings enabling shared growth, learning and development. You will become an active member of the Adult Community Services Team. Manage referrals into Adult Community Services and along with clinical support ensure onward referral to the appropriate Adult Community Service. KEY RESULT AREAS/MAIN DUTIES AND RESPONSIBILITIES: · Plans and manages own work load on a daily basis Ensures an accessible and facilitates a responsive service for patients and referrers to Adult Community Services Acts as the first point of contact for patients and referrers, responsible for managing telephone calls and emails, dealing with all enquiries, maintains detailed messages on electronic system and escalates to clinical staff as required Displays courteous, efficient and robust lines of communication between patients, community staff and other clinicians / external stakeholders e.g. community hospitals, voluntary sector, GP practices, hospitals, hospice, Disseminates service information to staff across the relevant team / designated area as requested using various means of communication: telephone, letter, e-mail, face to face meetings Books patient appointments / visits and patient transport booking as required using electronic EMIS system Maintains filing systems, ensuring unnecessary papers are disposed of in line with policy and information governance requirements Maintains up to date information on training and dates of training for all staff within the team / designated work area Reports all complaints, hazards, near miss incidents, incidents and accidents and escalates to line manager / clinical lead in a timely manner Utilizes electronic systems and databases as relevant Maintains data bases and reports at all times by entering data / patient information in a timely manner Your base will be at Milford Community Hospital which is located in beautiful rural surroundings and has free on-site parking Milford hospital is close to a main line station Full NHS pension scheme, terms and conditions apply.
