Role Overview
PWE is a group of three GP Practices which sit under the management of East Lancashire Teaching Hospitals (ELHT). Staff within the practice are employed by ELHT, a relatively new but increasingly common arrangement supporting Primary, Secondary and Community Care to collaborate in the best interests of the patients. This is an exciting opportunity to join our busy GP Practice in the delivery of nursing to our patients. Working as part of a multidisciplinary team delivering a variety of care to the patient population, responsible for a number of clinical areas such as Health Promotion, Chronic Disease Management, Ill Health Prevention, Well Woman and Well Man Clinics and other specialisms within your scope of practice. The role is central to the delivery of care to the practice population and will be a key member of the multidisciplinary team.
Responsibilities
This is an opportunity to make a real difference to the lives of our patients. Although this post is based at Pendle Valley Mills, Yarnspinners there may be occasions where the successful candidate is asked to provide cover at one of our alternative sites. It is crucial that applicants have a flexible approach to work with the willingness and ability to work as part of the wider team.

Qualifications
Required Qualifications
* Current NMC Registration
* Evidence of Immunisation Training with recent assessment of competence
* Ability to deliver a high standard of evidence based patient care
Desirable Qualifications
* Post Graduate Qualification
* NMC Assessor accreditation, Minimum of 6 months relevant primary care experience
* Clinical knowledge and experience of Immunisation, Venepuncture, Women's Health, Diabetes, Hypertension, Cardiovascular Disease, Coronary Disease, Cytology
* Experience of working autonomously within scope with the confidence to understand when to seek support
* Experience of Chronic Disease Management, Travel medicine, Mental health, ECG, Chaperoning
* Experience of requesting diagnostics, i.e. pathology and of interpreting the results and taking appropriate action
* Experience of using health promotion strategy
* Understanding of PGDs and associated policy
* Excellent Communication Skills – written, face to face and telephone
* Strong IT skills – Outlook, Office, EMIS
* Competent in Document Management Systems
* Effective time management
* Ability to work both autonomously and as part of a team
* Ability to follow clinical policy and process
* Experience of audit
* Experience of clinical risk management
* Leadership and Management skills
* Evidence of research and development, Polite and Professional approach
* Flexible approach to the role
* Forward thinking, motivated and committed to quality
* Ability to work under pressure whilst prioritising the needs of the patient

Salary and Hours
* Please note that the salary advertised is for full time hours. If this post is less than 37.5 hours per week, the salary will be pro‑rata.
