We are looking for an enthusiastic and experienced Band 6 Staff Nurse or Therapist, with specialist knowledge and experience in discharge planning and pathways, to join our Community Transfer of Care Hub based at Gravesham Community Hospital.
In the Community Transfer of Care Hub, you will manage all referrals into the Discharge to Assess Pathways across Dartford, Gravesend, and Swanley. You will work alongside a talented multi-disciplinary team of discharge nurses, therapists, rehabilitation and healthcare assistants, and an administrator to assist in discharge planning across acute hospital services.
Gravesham Community Hospital is located near St Georges Shopping Centre, surrounded by food establishments and coffee shops, making it an ideal location.
While based at Gravesham Community Hospital, travel to local hospitals to provide in-reach support and visit patients at home and in care homes for assessments in line with our "Home First" approach is required. A driving license and access to a car are essential.
Main duties of the job
* Support and manage the Community Transfer of Care Hub Team, providing clinical/management leadership, support, and guidance.
* Support recruitment efforts, participate in inductions, and involve patients and carers in planning community services.
* Maximize clinical resources, monitor patient flow, and support safe, timely discharges.
* Lead on complex cases, including assessments for homeless patients and review cases requiring 1-1 funding.
* Address safeguarding issues and develop effective communication with all relevant agencies.
* Take clinical responsibility for placements and resolve arising issues.
