Healthcare Assistant - End of Life Urgent Care Service
Hours: Part‑Time 34.5 hours per week
An exciting opportunity for an enthusiastic, well‑motivated individual interested in end‑of‑life care. St Luke’s is delivering its vision of a hospice without walls with the support of its End‑of‑Life Urgent Care Team. As a Healthcare Assistant for the End‑of‑Life Urgent Care Service you will assist in supporting community‑based end‑of‑life patients in meeting their preferred place of care or death and prevent inappropriate admission to secondary care.

Main duties of the job
Assist community‑based patients to maintain their preferred place of care, provide palliative support and comfort measures, coordinate with the multidisciplinary team, administer basic care interventions, and ensure accurate documentation and communication as per the care pathway.
