Community Palliative Care Nurse Specialist, Band 6

The community specialist palliative care service is looking for an enthusiastic, motivated and experienced registered nurse to join the team. The role is 30 hours a week, 9am‑5pm over four working days, and requires working as an autonomous practitioner to promote and maintain quality of life for patients with life‑limiting conditions in the primary care setting.
Main duties of the job
As a clinical nurse specialist you will manage a caseload of patients with complex symptoms and needs, providing person‑centred coordinated care with a cohesive, holistic and evidence‑based approach.

Job responsibilities
* Assess patients and carers autonomously within the community, addressing complex physical, emotional, social and psychological symptoms and developing an appropriate care plan.
* Provide specialist clinical advice to health care professionals, patients and carers on care and treatment, including medication, psychological issues and further treatments.
* Maintain effective communication in complex, sensitive and potentially stressful situations, breaking bad news, supporting relatives, carers and staff in bereavement and recognising and referring for complicated grief reactions.
* Identify, organise and evaluate specialist interventions in response to complex needs, including oncological, medical and palliative care emergencies, analysing clinical options and liaising with the multidisciplinary team.
* Contribute to case conferences and the multidisciplinary team to enable complex and ethically challenging decisions regarding treatment plans.

Person Specification
Qualifications, Knowledge, Skills & Experience
* Significant in‑depth NHS experience, with appropriate contact with palliative care, oncology or community nursing.
* RGN with substantial experience post registration.
* Excellent interpersonal and communication skills.
* Experience optimising and managing symptoms.
* Understanding of specialist palliative care and its difference in provision from other palliative and end‑of‑life care providers.
* Ability to work independently and as part of an integrated team.
* Critical thinking skills and receptiveness to constructive feedback for professional growth.
* Evidence of completion of Advanced Communication Skills Training or equivalent.
* Evidence of formal palliative care/symptom control training.
* Evidence of working within a community setting / lone working.
* Evidence of clinical leadership.
