We are looking for an enthusiastic and experienced neurological Physiotherapist to join our Stroke Early Supported Discharge Service working in the community setting across Buckinghamshire. This role is is a joint team lead role working closely with an Advanced Occupational Therapist to support, manage and develop a team of therapists and support workers whilst also provided specialist therapy and advice to stroke patients in their own homes. This role will suit someone who is passionate about providing high quality care and rehabilitation for stroke patients, working together with the MDT and in partnership with patients and families.
The post holder will be supported by the Stroke Early Supported Discharge Service lead, who is also a clinical specialist Physiotherapist.
Main duties of the job
To be responsible for jointly leading the Stroke early Supported Discharge Service Team providing a service to stroke patients in their homes immediately following Discharge from hospital.
To liaise closely with the stroke rehabilitation bridging service and community neuro rehab services to promote an efficient and effective community stroke service.
To perform specialised assessment and treatment for patients with complex presentations and needs, and to provide expert advice, guidance and information to health and social care professionals, carers, relatives and others.
To undertake all aspects of clinical responsibilities as a skilled, autonomous and advanced practitioner.
To actively lead development of professional and service initiatives, and contribute to organisational objectives.
To supervise, teach and appraise Physiotherapy, Speech and language therapy, Occupational Therapy staff and students within the specialty.

EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
10. Current registration with Health Professions Council
11. MCSP Membership
12. Diploma /Degree in Physiotherapy
13. Membership of professional special interest group
14. Masters degree in related field
15. Field work/clinical educators course
16. Evidence of CPD in the form of a detailed personal development portfolio
EXPERIENCE
Essential criteria
17. Minimum of 3-4 years working experience at Band 6 level in the NHS
18. Minimum of 18 months experience in related field at senior level (Acute Neurology, Stroke Rehab)
19. Experience in student clinical supervision
20. Attendance at post graduate courses in related fields
21. Experience in supervision of junior staff
22. Ability to work and interact effectively within an interdisciplinary team
23. Evidence of commitment to continuing professional development, and reflective practice
24. Knowledge and application of evidence based practice and clinical reasoning
25. Participation in audit and research activities
26. Establish and maintain CPD Portfolio
27. Demonstration of having initiated change of practice or process following identification of clinical or service need.
Desirable criteria
28. Masters degree in related field
