Job Title: Social Worker – Adult Services Team
Location: Bristol
Rate: £32 per hour
An exciting job role has recently become available at the Bristol City Council. They are looking for a new member for their Adult Services team.
What will your responsibilities be?
In this role, you will engage in a fast-paced, multi-disciplinary environment, working closely with health colleagues and community agencies. You will demonstrate a thorough working knowledge of hospital pathways to support patient care. Your responsibilities will include conducting Care Act Assessments, Mental Capacity Assessments, and chairing Best Interest Meetings. You will carry out timely reviews as required, ensuring ongoing support for patients. Additionally, you will apply a Multi-Disciplinary Team (MDT) approach to ensure a safe and efficient discharge from the hospital that aligns with the service user’s needs.

Qualifications and Experiences:
To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a full UK driving licence, possess post-qualified experience, and have a degree level or equivalent qualification in Social Work. These qualifications and experiences will ensure you are well-prepared to meet the demands of the position and provide high-quality support to those in need.
4. Proven experience in adult services within hospital settings.
5. Excellent communication skills to effectively collaborate with a team and external partners.
6. Proficient in completing assessments and reviews with precision and empathy
