Open Door is seeking a Registered Nurse (RN) for its Family Medical Center in Sleepy Hollow, NY. The Registered Nurse will promote and restore patient’s health by completing the nursing process, collaborating with the physicians to provide comprehensive quality care and support to patients and family. He/she will participate in cross-functional care teams.
Responsibilities:
1. Performs and accurately records screening tests (blood pressure, temperature, pulse, weight, EKG, vision, hearing) to patients of neonatal, pediatric, adolescent, adult and geriatric age groups.
2. Encourages patients to continue under medical supervision and aid them to carry out medical and social work recommendations.
3. Establishes a compassionate environment by providing emotional and psychological support to patients, friends, and families.
4. Identifies individual and community problems and makes appropriate referrals in coordination with team members.
5. Participates in patient counseling and education activities related to primary health care, HIV/STD prevention and treatment, child/adult health maintenance, perinatal and family planning.
6. Documents necessary information in patient’s electronic medical record.
7. Updates the practice management information system with all necessary clinical information for each patient prior to close of business day.
8. Prepares and maintains documentation in compliance with Joint Commission standards and applicable city, state and federal laws.
9. Administers medications and treatments as authorized by the provider.
10. Performs simple laboratory procedures including but not limited to rapid strep test, urinalysis, pregnancy testing, and tuberculosis screening.
11. Encourages preventative care to patients of all ages.
12. Maintains patient confidentiality at all times as per HIPAA and Open Door policies and procedures.
13. Reviews patient record one-day prior to scheduled visit to ensure chart preparedness.
14. Maintains current up to date immunization record through NYSIIS.
15. Follows current ACIP guidelines.
16. Participates in the storage and maintenance of medications and immunizations.
17. If Designated as responsible for Bio Hazardous Waste removal, complete online training and receive certificate prior to completing this task.
Requirements:
18. Associate RN is required
19. Current NYS RN license
20. 1+ year experience in community health
21. 2+ year’s previous experience in nursing preferred
22. Bilingual (Spanish) Preferred
23. Current CPR Certification
24. Outstanding written and verbal communications skills
25. Ability to work in a culturally diverse patient and staff environment
26. Ability to work in a collaborative team-based care environment
27. Highly organized, flexible, and self-directed, able to multi-task
28. Responsive and comfortable working in a fast paced environment
29. Excellent judgment skills and ability to prioritize tasks
