Clinical In-reach Occupational Therapist

An opportunity has arisen for a highly skilled, enthusiastic and motivated Occupational Therapist to join the management team of Integrated Care across Greater Nottingham.

FDIDT are an in-reach multidisciplinary team made up of AHPs, Nurses and Assistant Practitioners. This Nottingham CityCare Partnership service provides innovative, successful and patient focussed outcomes for those that present in the emergency and urgent pre admission areas at Nottingham University Hospitals QMC site.

Your role within the busy and dynamic team will be to provide a comprehensive holistic assessment of need with the aim of preventing avoidable hospital admissions.

The successful candidate will be able to work within the MDT as an autonomous practitioner, undertake comprehensive assessments, and complete profession specific assessment and evaluation as required, to identify the appropriate discharge plans for adults with a wide variety of complex physical and mental health needs.

The Teams Mission Statement is to provide a gold standard assessment and review of patient needs that allows ongoing treatment to be provided by the right service, at the right time, in the right place without delay or duplication. This is underpinned by our organisational aim of building healthier communities and being committed to the development and future of an Integrated Care System across Nottinghamshire through active membership of the local integrated care provider partnership.


Main duties of the job

* The post holder will be working as a multi-disciplinary team member to undertake comprehensive assessment and intervention to clients within the Pre-admission areas of Nottingham University Hospitals Queens Medical Centre Campus in order to facilitate the clients discharge or appropriate pathway identification.
* They will need sound knowledge and experience of working with complex, frail and older people that are able to manage a caseload that consists of a wide range of cases for people presenting multi pathologies, as well as challenging medical and social circumstances.
* Perform holistic assessments and a range of skills within a competency framework, the post holder will be expected to complete initial assessments and follow ups under the guidance and supervision of the Clinical staff.
* Utilise community knowledge and skills whilst assessing and recommending appropriate discharge support.
* Have excellent assessment, communication skills and intervention skills to be able to plan, develop and evaluate goal focussed rehabilitation.
* Able to prioritise workload, manage their time effectively, and work flexibly within the team.
* They will have ongoing supervision from a clinical member of staff directed and guided by your needs and be expected to participate in staff development programmes.

Job responsibilities

Job Purpose

* To work as an autonomous practitioner, you will undertake comprehensive assessments to identify the appropriate service for the clients referred.
* To carry out profession specific assessment and evaluation as required.
* To work within the MDT to plan and develop discharge plans for adults with a wide variety of complex physical and mental health needs
* To be responsible for continuing the on-going development of high standards of clinical practice with evidence-based focus within the service.
* To work, as required within the integrated care and Front Door Integrated Discharge services, reacting to emergency health and social care referrals with the aim of preventing admission to hospital.

Dimensions

* This post has no budgetary responsibility; however, the post holder is responsible for demonstrating cost consciousness.
* You will be required to work primarily with the Queens Medical Centre but with potential to work across different bases and community settings which may include clients own home, residential, nursing homes or hospital.
* You may be responsible for designated caseloads of clients
* You will be required to participate actively in City Cares training requirements.
* You will supervise, educate, and assess the competency of junior staff, support staff and students as required.

Key Responsibilities

Clinical

* To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care.
* To have the ability to gain valid informed consent and to identify through assessment clients who lack capacity and work within a legal framework with clients who lack the capacity to consent to treatment.
* To undertake the comprehensive assessment and evaluation of the patients Physical and Mental health needs to develop individualised discharge plans/rehabilitation/reablement goals.
* To undertake specific professional assessments to ensure the best outcomes for clients
* To use clinical reasoning and a wide range of skills and knowledge to devise specialised programmes of care.
* To be responsible for coordinating the safe and timely discharge process from a variety of settings, including hospital and community beds.
* To undertake comprehensive evidence-based risk assessments developing risk management plans.
* To provide specialist advice, education, and mentorship to other members of the team and colleagues regarding the clinical management of clients.
* To actively engage with families and carers in order to ensure the best outcome for the service user.
* To demonstrate flexibility, prioritise effectively and time manage own clinical workload. Be able to delegate appropriately.
* To lead in regular team meetings and review meetings regarding client caseload.
* To keep accurate and timely service user records to ensure continuity of care.
* To participate in the supervision framework as a supervisee and a supervisor, including the appraisal process.

Professional

* To maintain a high standard of care to clients and carers, demonstrating a commitment to evidence based practice.
* To be responsible for own Continuing Professional Development maintaining an up-to-date portfolio.
* To work to relevant Code of Professional Conduct.
* To contribute to Clinical Governance agenda and apply Risk Management strategies.
* To contribute to implementation of service developments utilising enhanced knowledge and skills.

Organisational

* To support the Team Manager in implementing and evaluating targets and objectives to assist in the ongoing development of the service.
* To advise the Team Manager in relation to clinical needs of the client group.
* To take delegated responsibility for management duties as required.
* To always ensure the health & safety of self and others in accordance with Health & Safety legislation and City Care policy and procedures.
* To use information technology daily and participate in identified data collection and statistics and maintain clear, concise, and timely records in line with the service policy.
* To participate in the Lone Worker Tracking procedure as applies to the team.
* To act as duty clinician for the process of referrals.
* To undertake any other duties which may be reasonably required.

Education and Leadership

* To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Also, to keep an active up to date CPD portfolio.
* To participate in the education of colleagues, junior staff, students, and multi-professional teams using excellent mentoring and presentation skills.
* To assist in the training and development of others (new members of staff, students, and visitors to the service).
* To keep up to date with IT skills in line with the modernisation of data collection records.
* To participate in appropriate in-service training and individual learning with specific and measurable outcomes.


Person Specification


Knowledge and skills

* Understanding the concepts of assessment, completing standard assessment of need, able to formulate a care planning and taking appropriate action.
* Awareness of a range of activity of daily living equipment and adaptations.
* Effective communication skills (Verbal and written) and good interpersonal skills and IT skills.
* Ability to involve individuals in a person-centred approach to care.
