Responsibilities
* Conduct face‑to‑face assessments and reviews in the community, ensuring timely and effective support for vulnerable adults.
* Lead risk assessment and complex case management, prioritising safeguarding and health‑and‑wellbeing needs.
* Work collaboratively with multidisciplinary teams to develop and implement individual care plans.
* Provide practical support under the Care Act 2014 and the Mental Capacity Act 2005, and participate in related audits and training.
* Travel to service users’ homes and community sites as required.
Qualifications and Experience
* wide safeguarding experience and ability to work independently and as part of a team.
* proven risk assessment and complex case management skills.
* strong knowledge of the Care Act 2014 and the Mental Capacity Act 2005.
* excellent communication and customer‑service skills.
* resilience and effective change‑management under pressure.
