The Role:
Help People Thrive at Home!
When someone leaves hospital, everyday tasks can feel overwhelming. As a Community Reablement Assistant, you’ll help people regain their confidence and independence in their own homes - supporting them to live life as fully and independently as possible.
Our Rapid Response service is available from 07:00 to 22:00, every day of the year, enabling us to deliver flexible, timely support whenever it’s needed most. You’ll work on a fixed 3-week rolling rota, so your shifts will include evenings, weekends, and public holidays - for which you’ll receive enhanced pay in line with HCC’s salary policy. You’ll cover the West Hampshire area which includes places such as New Forest, Eastleigh, Totton, Hythe and Romsey.
What you’ll do:
Travel to service users’ homes to:
* Support Daily Routines - Help with washing, dressing, nutrition and continence care.
* Deliver Person-Centred Care - Tailor your support to individual needs.
* Promote Independence - Encourage and enable people to do things for themselves again.
* Provide Reassurance - Motivate and support people through recovery.
* Build Relationships - Work closely with service users, families and your team.

We have part-time positions available at 18.5 and 25 hours per week:-
* 18.5 hour role actual salary is £12,593 - £13,122 per annum
* 25 hour role actual salary is £17,018 - £17,733 per annum

Travel Requirements:
As the role involves visiting people in their own homes, you’ll be required to travel to various locations within Hampshire and it’s essential that you have a current drivers’ licence and access to a reliable vehicle on a regular basis.
You’ll be able to claim your business travel mileage to and from your home address in line with our expenses policy.
You may occasionally be required to travel outside of the West Hampshire area. Full information on the areas we cover will be provided at interview stage.
