What You’ll Be Doing
* Carrying out strengths-based assessments and delivering tailored care plans.
* Managing a varied caseload within a multi-disciplinary team.
* Building meaningful relationships with adults and their families.
* Offering support, guidance and advice to unqualified staff and students.
* Ensuring interventions deliver sustainable, positive outcomes.
* Applying your knowledge of legislation and best practices in adult care.
? What We’re Looking For
* Qualified Social Worker registered with Social Work England.
* 2-4 years’ experience in Adult Social Care (assessment or long-term).
* Strong communication, mediation, and interpersonal skills.
* A creative and solution-focused approach.
* Confident in multi-agency working and risk assessment.
* A genuine passion for supporting adults and promoting independence.
