Team Manager – Adults Social Care (Hospital Discharge) Location: West Yorkshire

Are you an experienced Team Manager with a background in Adults Social Care and hospital discharge? Ready to take on a vital leadership role that directly supports safe and person-centred care for adults leaving hospital? Pertemps Professional Recruitment is seeking a skilled and motivated Qualified Social Work Team Manager to lead a dedicated Hospital Discharge Team in West Yorkshire. You’ll be at the heart of operations, driving quality practice, managing complex casework, and supporting your team through high-pressure frontline work.
Your Role Will Include:
1. Leading and supporting a team of Social Workers and practitioners delivering hospital discharge planning and safeguarding work.
2. Overseeing Care Act assessments, capacity decisions, and case management.
3. Managing a duty function within the team and ensuring timely, effective responses to safeguarding concerns.
4. Working closely with NHS, health professionals, and partner agencies to ensure seamless multi-agency working.
5. Ensuring your team’s compliance with relevant legislation including the Care Act 2014, Mental Capacity Act, Human Rights Act, and Mental Health Act.
6. Maintaining a strong presence across hospital sites and local offices to support staff and ensure operational excellence.
What We're Looking For:
7. Qualified Social Worker, registered with Social Work England.
8. Proven leadership/management experience in adults' services—especially within hospital discharge or safeguarding teams.
9. Excellent understanding of current adult social care legislation and statutory responsibilities.
10. Strong people management and supervision skills, with the ability to motivate and support team members.
11. Confidence managing complex cases and risk in a fast-paced environment.
12. A commitment to high-quality, person-centred care and positive outcomes for vulnerable adults.
