Supported Assistant Practitioner/Assistant Practitioner 8673 | Permanent contract | £28,598 to £33,699 per annum (Scale F/G) | 37 hours per week | County Hall, Norwich
The Preparing for Adult Life (PfAL) Service ensures young people moving into Adult Social Care are supported to do so in a seamless, joined up way.
You will have the ability to work with young people with complex needs (including those with Learning Disability, complex mental health needs or physical disabilities, young people who are Autistic), as well as their families. You will be able to demonstrate an understanding of the legislative requirements in relation to the Care Act 2014, as well as other awareness of other legislation including the Children and Families Act 2014 and Mental Capacity Act. As part of this role you will be expected to carry out assessments and care planning under the Care Act.
