An exciting opportunity has arisen within the Department of Older Persons Services at Great Western Hospital NHS Foundation Trust. We are looking for a qualified Advanced Clinical Practitioner (ACP) who is dynamic, enthusiastic, and passionate about providing high-quality care tailored to the needs of older patients.
This is a 12-month fixed-term position to cover maternity leave.
The Older Person ACP will support the transformational development of a comprehensive, holistic, evidence-based multidisciplinary service. The successful applicant will work closely with the Matron, Head of Service, and Clinical Lead to proactively manage older adults in the acute setting.
Your role will involve implementing an improved and consistent approach to assessing and identifying patients with frailty at the front door, ensuring they receive a comprehensive geriatric assessment. You will aim to reduce unnecessary ward admissions and length of stay through effective triage and assessment.
As part of a multidisciplinary team, you will develop your advanced clinical practice, striving for autonomy while practicing collaboratively and demonstrating accountability and self-direction according to professional standards. You will become a clinical leader in your area, fostering innovative solutions to improve patient care and experience.
The post requires demonstrating advanced clinical judgment, utilizing specialised equipment and diagnostic reasoning to make differential diagnoses. You will be capable of working effectively in high-pressure environments, ensuring safe patient care.
You will develop expertise within a specific area, enabling you to admit, discharge, review inpatients, and provide outpatient follow-up care, supported by a middle-grade medical colleague.
Upholding the four pillars of advanced practice, you will support and lead staff, ensuring high standards of care. Your proven skills in physical assessment, diagnostics, and prescribing will allow you to work autonomously and as part of the team. As a clinical leader, you will support daily safe and effective care delivery and assist senior management with projects and service improvements.

The Department of Older Persons Services (DOPS), part of the Division of Medicine, covers Front Door assessment areas and inpatient wards for older persons. We operate the largest bed base within GWH, providing care to a vulnerable patient group. Our services include a Frailty Hot Clinic twice weekly and comprehensive geriatric assessments to deliver person-centred care.
