Our new service integrates our Children Single Point of Access (CSPoA) team, the Missing and Exploitation team, the Supporting Families team, and combines the former Access & Assessment (A&A) and Family Intervention (FIT) teams into four Together for Families teams.
The Role
As a Together for Families Social Worker, you will hold a varied caseload and carry out the full range of statutory social work duties from initial assessment through to targeted intervention.
A key feature is that the same social worker will be allocated to a child or young person for the duration of the time their case is open to Children’s Social Care, until the case is closed or transferred to the Children Looked After Service. This means far fewer transitions for children and families and further develops our relational-based practice.
Key Responsibilities
* Carrying out Single Assessments, risk assessments, and initiating Child Protection procedures where necessary.
* Developing, implementing, and reviewing Child in Need (CIN) and Child Protection (CP) plans.
* Working closely with children and families to build a deep, continuous understanding of their needs and lived experiences.
* Providing ongoing support and intervention to families, applying principles of trauma-informed practice.
* Preparing and presenting reports for statutory reviews, case conferences, and court hearings as required.
* Collaborating effectively with partner agencies, including Health, Education, and Police, to deliver coordinated support
