A DAY IN THE LIFE OF A SUNFLOWER CARE AT HOME PRACTITIONER Reporting to the Sunflower Care at Home Manager, you can expect your working day to include the following: Supporting family members and other professionals to provide excellent holistic, person-centred, palliative care. Supporting Service Users with their physical and emotional needs. Assisting with personal care, toileting, bathing, dressing and meals WHAT WE NEED FROM YOU To succeed in the role of Care at Home Practitioner you will be: Experienced in providing exceptional, holistic, person-centred care and support. Passionate about helping people. Compassionate, caring, and resilient. Respectful, sensitive and have an empathic approach. An excellent communicator who is comfortable talking to and working with family members and people from different professions. You’ll be great at keeping records, too. Take a pride in what you do. SVQ 2 Health and Social Care or equivalent – or willingness to work towards this. Full Driving Licence and a vehicle to use. Dedicated and passionate about making a real difference to people’s lives.
