Cornwall Partnership NHS Foundation Trust has an exciting opportunity available for an enthusiastic and highly motivated individual to join our Integrated Community Mental Health Team based in the East of Cornwall.
We offer assessment and treatment of mental health problems, with a focus on effectiveness and recovery, working to promote the independence of our service users. The team are looking for someone who can demonstrate the Trust's vision and values in practice, are motivated to be involved in innovation and service development and passionate about mental health.
We are looking for those people who have a core profession in mental health nursing to join our community service line. Additionally, you must be keen to take on a challenging role in a busy Community Mental Health Team and have the ability to work in a self-directed way but also work well as part of a team.
Responsibilities
This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
To be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.
To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.
Essential Qualifications and Experience
* Current professional registration - NMC
* Professional qualification - RMN
* Evidence of post basic training
Desirable
* Teaching & Assessing in Clinical Practice
Knowledge and Skills
* Sound working knowledge of the Mental Health Act of 1983 and other mental health legislation
* Ability to supervise staff, demonstrating the highest clinical standards
Disclosure and Registration
