Social Worker – Learning Disabilities (Transitions Project) London Borough of Islington Up to £32 per hour About the Role Islington Adult Social Care is seeking a dedicated Social Worker to join our team for a targeted project. The primary focus is to complete a backlog of initial Care Act assessments for young people transitioning from children’s services to adult social care. You will work in a strengths-based way to ensure residents feel safe, valued, and supported to live the lives they choose. Key Responsibilities * Complete a backlog of initial Care Act assessments for young people in transition. * Utilize specialist knowledge of Learning Disabilities, Autism, and ADHD to provide high-quality support. * Carry out statutory responsibilities under the Care Act and Mental Capacity Act. * Prioritize delaying and preventing the need for formal intervention through strengths-based conversations. * Work in a "think family" approach, supporting families as a whole network. * Identify mental capacity and undertake Best Interest reports where required. * Maintain accurate, timely electronic case records in line with council policy. Requirements * Degree in Social Work and current registration with Social Work England. * Proven experience assessing needs in line with statutory responsibilities. * Knowledge of the impact of Learning Disabilities, Autism, and ADHD on young adults. * Commitment to challenging inequality and creating a fairer borough for all. * Ability to work effectively within a fast-paced multi-disciplinary team
