Overview
The Mid Yorkshire Stroke Service requires a consultant to complement the current team providing stroke services for the Wakefield District, north Kirklees, parts of south Leeds and the Hyperacute Services for large parts of Barnsley. This is a substantive post for Stroke Medicine. You will join a team of 4 substantive Stroke Physicians within the Department of Stroke Medicine, which is part of the wider Neurosciences team. The post supports the entire pathway, including Hyperacute services, the acute stroke unit and rehabilitation unit, and on-call duties for thrombolysis both out of hours and weekends. The consultant is also expected to support the TIA clinic which operates every day of the week.
Responsibilities
* Provide high-quality care to all patients admitted to the stroke service at MYTT.
* Keep patients and/or their carers informed about their condition and involve them in decision-making about treatment.
* Maintain professional standards as set by the General Medical Council (GMC) and comply with the GMC guidance on Good Medical Practice.
* Contribute to Urgent and Emergency Care duties, including out-of-hours on-call, in line with working time regulations and safe working practices.
* Share on-call duties with 7 other colleagues on a 1:8 basis on weekdays and weekends; out-of-hours on-call is provided via telemedicine to supervising trainees.
* Carry out administrative duties with access to consultant office space, secretarial support and IT facilities.
* Support the Trust framework through Providing Additional/Supporting Professional Activities (SPAs) with a minimum of 1.5 SPAs included in the core 10 PA timetable; additional SPA or DCC time may be commissioned as required.
* Work with the Stroke Service at MYTT, the HASU and the Stroke Rehabilitation Unit across the Wakefield, Dewsbury and local sites, including involvement in TIA clinics.
* Participate in teaching, departmental leadership or research activities as appropriate to the role.
Service context
MYTT is an Acute Stroke Centre providing HASU care for Wakefield district, north Kirklees, parts of Barnsley and south Leeds. Acute services are at Pinderfields Hospital in Wakefield; the stroke rehabilitation unit is at Dewsbury and District Hospital (DDH). There are currently 7 consultant posts (4 substantive and 3 locums) and a rotating junior doctor team. Stroke Assessment Hub admits over 1,200 confirmed stroke patients annually, with an additional 400–600 stroke mimics, supported by 2 Band 6+ Stroke Assessment Nurses 24 hours a day. Mechanical thrombectomy is not currently available 24/7; CTA/CTP are available within thrombectomy provisions and can be performed around the clock when required. Out-of-hours CT and thrombolysis decisions are made with junior staff under telemedicine supervision.
The HASU is 12-bedded with central monitoring; ASU is 39-bedded with daily ward rounds by two consultants and handover to the nurse coordinator. The Stroke Rehabilitation Unit is 16-bedded and co-located with the Neurological Rehabilitation Centre at DDH. The TIA service currently runs as a mix of face-to-face and non-face-to-face clinics, with some cases seen at the Hub for examination where necessary. The OP workload includes two clinics per week for TIA (5 new patients plus investigations) and the Stroke clinic (up to 3 new patients and 5 follow-ups with complex needs).
Qualifications
* Essential – Medical Degree from a recognized medical school; MRCP or equivalent; Full GMC Registration with licence to practise; inclusion on the GMC Specialist Register for Geriatric Medicine, Neurology, Rehabilitation Medicine or Clinical Pharmacology with completion of Stroke Fellowship (or demonstration of equivalent competences) or within 6 months of obtaining CCT/CESR.
* Desirable – Medical School prizes; audit or case presentations; teaching qualification/prize; computer literacy; specific educational interests with evidence of educational ideas or project work.
* Management skills – Demonstrates intelligent and analytical problem solving; good time management; organisational and personal management skills.
* Experience desirable but not essential – ability to contribute to formal teaching, departmental leadership or research activities.
