The role of the SCPHN School Nurse is to deliver the Healthy Child Programme to all eligible children and young people as part of an integrated team with Primary School Screeners and local service providers. This is achieved through individualised health needs assessments, planning of programmes of care tailored to children and young people's needs, and effective liaison with colleagues from partner agencies.
The post holder will work as a member of the School Nursing Service and will work with schools, families, and the wider community to improve the health of school-aged children, identifying the more vulnerable, and developing clear referral pathways so that all children and young people can reach their full potential.
This will be done by delivering the requirements from the service specification. They will work closely with the identified senior manager in each school providing expert information, guidance, and assessments as required.
They will support schools to empower parents, and CYP to make decisions that affect their health and wellbeing. They will work across health and education professionals and institutions, breaking down barriers to improve the sharing of information, ideas, and expertise.
