Upper Calder Valley PCN consists of two GP practices andhave a combined patient list of approx. 35,000 patients. We have an exciting opportunity for anexperienced pharmacist to join our established pharmacy team. We currently have five pharmacists and threepharmacy technicians working across the two GP practices. Our team work closely together and are highlyregarded and integrated team members that are well supported by the wider PCN. Theideal candidate will work well within a team and have good interpersonal skillsas well as being able to work as part of multi-disciplinary team.


Main duties of the job

As one of our clinical pharmacists you will work as a keypart of a multidisciplinary team supporting, promoting and implementing highquality, evidence-based, cost effective and safe use of medicines. You will useyour clinical medicines expertise to support both patients and general practiceteams and improve prescribing practices across the PCN through mechanisms suchas process reviews and clinical audits.You will conduct structured medication reviews to proactively managepatients with complex polypharmacy. Youwill assist in repeat prescribing and medication reviews across both practicesand will have the opportunity to work in an establish pharmacist-ledhypertension clinic. As part of acollaborative team, you will work with other pharmacy colleagues across thePCN, including the ICB pharmacy team to ensure an effective and integratedapproach to medicines optimisation for our population

Job responsibilities

Workingas part of a multidisciplinary team in a patient-facing role.

Support,promote and implement high quality, evidence-based, cost-effective, and safeuse of medicines.

Supportdelivery of the new network Directly Enhanced Service (DES) specifications.

Conductstructured medication reviews to proactively manage patients with complexpolypharmacy.

Provideclinical medicines expertise to both patients and general practice teams.

Improveprescribing practices across the Primary Care Network.

Improveservice and quality through mechanisms such as clinical audits.

Interfacewith community and hospital colleagues to help resolve medication-relatedissues.

Assistwith delivery of enhanced health in care homes initiatives across the PCNfootprint.

Workcollaboratively with other pharmacy colleagues across the PCN footprint,including Clinical Pharmacists and Pharmacy Technicians, to ensure andeffective and integrated approach to medicines optimisation for our population.

Performface to face medication reviews for patients with polypharmacy, in practice andin patients homes (including care homes).

Throughstructured medication reviews, support patients to take their medications toget the best from them, reduce waste and promote self-care.

Provideexpertise in clinical medicines review and address public health and socialneeds of patients in GP practices.

Discusscomplex and sensitive information with patients during medication reviewincluding medical histories, lifestyles, and personal circumstances and torefer on to other services and professionals as appropriate.

Improvepatient and carer understanding of confidence in and compliance with theirmedicines.

Provideleadership on person-centred medicines optimisation and quality improvementwhilst contributing to the quality and outcomes framework and enhancedservices.

Providegeneral practitioners with appropriate recommendations and action plansfollowing medication review clinics with patients to optimise the patientstreatment with medicines and ensure appropriate follow up with patients.

Recordmedication review plans, recommendations and outcomes on GP computer clinicalsystems and own clinical notes in accordance with practice procedures.

Reconcile medicines following discharge fromhospitals, intermediate care and into care homes; identify and rectifyunexplained changes; manage changes by referral to the GP perform clinicalmedication review; support the production of post-discharge medicines careplans and work withpatients and other pharmacy colleagues to ensurepatients receive the medicines they need.

Supportthe processing of complex repeat prescribing medication requests made bypatients or other repeat prescribing queries.

Workin partnership with hospital clinicians to proactively manage patients at highrisk of medicine-related problems before they are discharged to ensurecontinuity of care.

Liaisewith the Practice Clinical team on prescribing and medication issues,especially around management of long-term conditions.

Developas an independent prescriber to manage and prescribe for patients within aspecific chronic disease area and within own areas of competence.

Withappropriate training, monitor the effects of medication by checking clinicalsigns (e.g., BP, pulse, etc).

Attendregular clinical supervision sessions to promote a culture of continuousprofessional development and shared learning.

Aspart of the multi-disciplinary team, attend relevant MDT Network meetings,providing information and feedback.

Reportmedicines related incidents, contribute to investigations and root causeanalyses as well as participating in serious incident investigations andmultidisciplinary case reviews.

Delivertraining, mentoring and guidance to other clinicians are kept up to dateregarding changes in national and local medicines guidance and legislation.

Criticallyappraise clinical evidence and present information to prescribers viaprescribing meetings and educational events, as required.

Leadon medicines management governance arrangements to adhere to CQC standards,including assisting in developing policies and SOPs.

Reviewrepeats prescribing protocols and processes and support staff in theirimplementations.

Performprescribing audits and direct/supervise the audit activity of others.

Facilitategood working relationships between practices and local community pharmacists.

Beresponsible for delivering local prescribing quality initiatives incollaboration with CCG medicine management.

Developan effective network and working relationship with individual practice GPs,practice staff, CCG medicines management colleagues, secondary care, and otherprofessionals.


Person Specification


Experience

* Knowledge of IT systems and ability to grasp new systems quickly.
* Evidence of experience as a pharmacist demonstrated.
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
* Experience of patient consultation.
* Experience of working with GPs
* Evidence of Continued Professional Development.
* General Practice and/or PCN experience.
* Experience of partnership/collaborative working and of building relationships across a variety of organisations.
* In depth, therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
* Understanding of the wider determinants of health, including social, economic, and environmental factors and their impact on communities.
* Knowledge of General Practice clinical systems, such as, EMIS and SystmOne. Able to plan, manage, monitor, and review general medicine optimisation issues in core areas for long term conditions.
* Commitment to reducing health inequalities and proactively working to reach people from all communities.
* Ability to work flexible and enthusiastically within a team or on own initiative.
* Is able to recognise personal limitations and refer to more appropriate colleague(s) when necessary.
* Able to work under pressure and meet deadlines.
* Ability to maintain effective working relationships and to promote collaborative practice with all colleagues.
* High level of written and oral communication skills.


Qualifications

* Mandatory registration with General Pharmaceutical Council
* Minimum of 2 years post qualification experience
* Non-medical Prescriber Registration
* Completed or in progress CPPE pathway
