Role Overview: We are seeking a highly motivated and experienced Primary Care Pharmacist to join a dynamic team in Leeds on a temporary basis through an agency contract. The successful candidate will play a crucial role in providing pharmaceutical care to patients, ensuring the safe and effective use of medications, and supporting the overall healthcare team.
Key Responsibilities:
* Conduct medication reviews and provide expert advice on the use of medicines.
* Collaborate with healthcare professionals to develop and implement patient care plans.
* Monitor patient progress and adjust treatments as necessary.
* Educate patients and carers on medication management and adherence.
Requirements:
* Registered Pharmacist with the General Pharmaceutical Council (GPhC).
* Minimum of 2 years' experience in a primary care setting.
* Strong clinical knowledge and excellent communication skills.
* Ability to work effectively within a multidisciplinary team.
