Job Summary

An exciting opportunity has arisen for a band 5 staff nurse to join our Rapid Response Team.

The rapid response team provides multi-professional input for patients living within the Gateshead locality, aiming to support patients at home to prevent deterioration in health and wellbeing and to be as independent as possible, avoiding the need for admission to hospital or long term care and supporting earlier hospital discharge where required. The team also provides out of hours palliative care support.

You will be based at Bensham Hospital and will be required to work as part of a rota to cover the service which operates 24/7.

Ideally you will have experience of working in the community and have experience of managing patients with a range of physical health problems.

You will be required to drive to a number of community locations as part of this role.

We are looking for enthusiastic, well-motivated, innovative individuals to support developments within the service.

Successful applicants will need to have effective communication skills and demonstrate a positive flexible, team working approach.

You will receive regular support to develop you both your clinical and professional skills and will have opportunities to contribute to the learning and development of other staff/students.

Main duties of the job

As a Staff Nurse you will be part of a community workforce providing interventions to the patients across Gateshead, preventing hospital admissions, enabling individuals to remain in their own homes for longer and prevent premature admissions into long term care facilities.
The post holder will assist individuals and their carers to proactively manage and anticipate their future health needs, support self-care, develop appropriate plans, and work collaboratively with the wider multidisciplinary team, partner organisations
Provide high quality patient care in the community setting under the direction and supervision of a senior nurse/practitioner.
To support the point of contact ensuring patients receive care at the right time, right place, by staff with the right skills.
Provide hands on care to patients in a variety of settings within the community
Treat specific diseases in accordance with agreed medical protocols, pathways and PGDs under direction of the senior practitioner.
Assess, plan, provide and evaluate nursing care to meet care needs of individual patients and carers in their own homes.
Analyse and evaluate personal clinical performance in a reflective manner to ensure continued personal and professional development and the maintenance of an up-to-date knowledge base.
Work with Trust policies and procedures that ensure patient safety at all times

Job responsibilities

As a Staff Nurse you will be part of a community-based workforce providing timely interventions to the patients across Gateshead, preventing unnecessary acute hospital admissions, enabling individuals to remain in their own homes for longer and prevent premature admissions into long term care facilities.

The post holder will assist individuals and their carers to proactively manage and anticipate their future health needs, support self-care, develop appropriate plans, and work collaboratively with the wider multidisciplinary team, partner organisations such as NEAS, GP/ primary care, pharmacy services and wider Integrated Locality Team members.

The post holder will support the team lead in delivering and evaluating high quality patient centered services in conjunction with the associated clinical teams to ensure this is incorporated within the Community Business Unit and Trust objectives, in line with Local and National Guidelines.

Be responsible for delivery of an identified unplanned or unplanned intervention under the supervision and direction of a senior member of staff

Provide high quality patient care in the community setting under the direction and supervision of a senior nurse/practitioner.

To support the point of contact ensuring patients receive care at the right time, right place, by staff with the right skills.

Provide hands on care to patients in a variety of settings within the community

Maintain standards of care through evaluating and implementing evidence based practice.

Delegate work appropriately according to skill mix ensuring maintenance of safety within the community.

Support the development of other staff and the service provided.

Work independently and under direction of the senior practitioner within a multi-disciplinary of an integrated workforce that is based in the community and is designed to meet the health needs of the patients in Gateshead

Deliver high standards of clinical practice that hold patients and their families at the centre to ensure that they receive safe, quality care that gives them a good experience.

Works as part of a multi-disciplinary team to deliver the best outcomes for patients and their families.

Assesses plans implements and evaluates clinical packages of care.

Deliver hands on clinical care for a defined population of patients.

Contributes the service delivery to CIP/CQUIN indicators and other performance and quality indicators within the Rapid Response Team as designated by the Rapid response Team Lead

To motivate and support the team approach to delivering new innovative models of care for patients across pathways and across providers as defined by the community Services Business Unit and Rapid Response Team Lead.

Ensure systems and processes are in place to maintain patient safety in line with choose safer care program in collaboration with the Rapid Response Team Lead and the BU Clinical Lead

Ensure and contribute to the experience of our patients and their families and ensure this is understood within all community teams.

Supports the senior team and contributes to workforce development in line with team profile and national nursing and AHP landscape

Supports the development of clinical expertise and advances in Nursing and AHP practice within the team in collaboration with the Rapid Response team lead, Clinical Operational Manager and Clinical Lead.

Supports the Team Leader in continuous service improvements and support the delivery of transformational projects within the Community Services Business Unit.

To ensure the development and maintenance of a learning environment within the multi-disciplinary team that is appropriate for the development needs of all members.

Contributes to new staff induction process adhering to and following standards, competencies and on-going education.

To establish an effective network of communication, which encourages a two-way dialogue

To facilitate an environment where health promotion and informed patient choice is encouraged and developed to its maximum potential, within the available resources.

To contribute to the evaluation of clinical practice within the area of community nursing by monitoring standards of care and ensuring that nursing practice is audited and research based, and supports the clinical governance framework.

The post will be based in Gateshead. The post holder will be expected to travel to patients living within the borough of Gateshead and locally, if the patient is registered with a Gateshead GP.

The post holder will be expected to work flexibly covering a variety shifts to meet the needs of the service 24/7.
