Douglas Grove Surgery in Witham are on the lookout for a capable and confident practice nurse to join our primary care team. We are flexible re hours to be worked and happy to discuss this with interested applicants - in the region of 28 hours per week. To be considered, you must be experienced in the full scope of general practice nursing duties.
Responsibilities
* Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
* Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
* General health and well‑being promotion as well as creating individual, person‑centred plans for patients with, or at risk of, a long term condition.
* Managing and treating long‑term conditions, in line with national and local policies and the practice needs.
* Reviewing medication for therapeutic effectiveness, in accordance with evidence‑based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
* Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Supporting and managing the health needs of women presenting for family planning and cervical cytology consultations.
* Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
* Meeting the needs of patients presenting for opportunistic wound care.
* Hypertension review and management.
* Running asthma, COPD and diabetes clinics and reviews.
* New patient and health checks.
* Travel health advice and vaccinations.
* Infection control and health promotion.
* Support for QOF and enhanced services.
Douglas Grove Surgery is a busy well‑organised teaching practice, serving around 6500 patients. Supported by a multidisciplinary team including: GPs, nurse practitioners, practice nurses, HCAs, clinical pharmacists, FCPs, mental health nurses, care co‑ordinators, social prescriber. We are a friendly and supportive team that strive to provide an outstanding service to its patients.
