The PCN Community Nurse Practitioner is expected to maintain contemporary clinical competence and credibility, and to be closely integrated with the wider practice team and related services. Key responsibilities include: Reviewing and providing care for patients in the community, whether for preplanned services or same day urgent care. Diagnosing and treating patients presenting with minor illness. Managing ongoing medical and nursing needs of patients, including advice and follow up care. Prioritising patient needs appropriately, including arranging investigations and making referrals when necessary. Any patient assessed as requiring secondary care referral should be discussed with the registered GP before the referral is made.
