Advanced Clinical Practitioner - Rapid Response Team
NELFT Waltham Forest would like to offer you a new and exciting opportunity to join the Waltham Forest Rapid Response Team. The team works across 7 days a week, 24 hours a day, providing urgent Multidisciplinary (MDT) response to prevent avoidable hospital admissions.
The aim is to support and treat suitable patients in the community, with a range of escalated health and social care needs. Waltham Forest is a diverse and multicultural borough in outer London with lots of green space and a thriving local market and independents shops.
Main duties of the job include:
* Working collaboratively with health and social care professionals across the borough and Integrated Care System.
* Leading Clinical decision making and undertaking enhanced holistic assessment of patient/family needs in partnership with the patient and their family/carer.
* Triage referrals from other agencies or disciplines.
* Co-ordinating intensive and complex nursing care with other multi-disciplinary agencies.
* Having management experience and acute care experience.
* Being proficient in advanced clinical skills and knowledge with the ability to assess the physical and psychosocial needs and develop a comprehensive and concordant plan of care.
* Working collaboratively and carrying out joint visits with MDT colleagues within the team to promote holistic person-centred assessment and approach to care.
* Participating and completing managerial and clinical supervision.
* Providing line management supervision to members of the Rapid Response multidisciplinary team.
* Contributing towards and carrying out audits within the service.
* Conducting and contributing to Datix investigations/ Root Cause Analysis/ Serious Incidents and to help support any lessons learnt within the service.
* Supporting and mentoring pre-registration learners from a variety of disciplines.
* Being a driver and having access to a car.
Person Specification:
Essential:
* Registered Nurse (Part 1 of NMC register) or Health & Care Professions Council eg Paramedic.
* Non-Medical Prescriber.
* Postgraduate level study or able to demonstrate post graduate level working evidenced via continuous professional development.
* Significant previous post registration experience in the relevant key areas at Band 6 or above.
* Experience of working in a community or urgent care response team.
* Experience of managing in a customer focussed environment, providing professional support within a busy environment.
* Demonstrate skills to organise, prioritise clinical caseloads within a multidisciplinary team.
* Demonstrate up to date knowledge and expertise in the speciality e.g. 0-2hr Urgent Care Response.
* Demonstrate a clear vision of the role and the service.
* Demonstrate proficient information, written and orally, in a clear and logical manner.
* Reliable and adaptable with excellent interpersonal and communication skill.
* Demonstrate compassionate and followership leadership skills.
Desirable:
* Certificate/Diploma in Management Studies Specialist Community Practice Teacher or equivalent.
* Experience of quality improvement processes and clinical audits.
* Demonstrate knowledge and understanding of the health needs of a culturally and ethnically diverse urban population with levels of deprivation.
* Proficient IT skills e.g. Word, Data entry or IT qualification or equivalent.
