Job Overview

The post holder will be responsible for delivering a robust integrated discharge model that ensures effective, person‑centred and integrated pathways of care out of hospital are embedded through interdisciplinary ways of working and clear, well‑managed processes and protocols. The Integrated Discharge Service is a collaborative and partnership approach with the Royal Free Hospital to coordinate and plan the transfer of patients, including those with complex health and social care needs, back to their usual place of residence wherever possible following an episode in hospital. The post holder will also work with University College London Hospital to support the discharge of Camden and Barnet patients and may be asked to take on additional responsibilities as national policy changes.

The post holder will be the lead for the Camden Community Access team, line‑managed by the Operational Support Manager, coordinating community health referrals within Camden. The post holder will provide visible leadership for the whole Camden Community Access and Integrated Discharge Service and directly manage the team’s strategic and operational responsibilities.


Main Duties

* Provision of operational leadership to the Integrated Discharge Team and Central Access team, oversight of the ICE Hub.
* Operational provision and delivery of the established IDT (Transfer of Care Hub) focusing on an integrated model to increase patient flow.
* Daily management (clinical and operational) and challenge and escalation of any delayed discharges.
* Problem‑solving and unblocking issues with partner organisations to ensure delays are resolved swiftly, escalating only where necessary.
* Escalation and reporting of issues relating to clinical risk.
* Initiating, leading and facilitating quality improvement and other projects within the department with stakeholders as necessary.
* Leading capacity planning and assurance processes for key events.
* Attending and providing system leadership to MADE and other events and meetings as needed.
* Providing accurate reporting data as required, including for governance forums.
* Monitoring, reporting and acting on key trends and themes identified.
* Responsible for policy implementation and service development across CNWL and partners.
* Development, sign‑off and implementation of Standard Operating Procedures incorporating agreed pathways out of hospital.
* Discharge Alerts: ensuring alerts are reported, reviewed and themes with learning and relevant action‑plan to improve discharges are carried out and shared via local governance meetings.


Qualifications and Training

* University degree or equivalent professional qualification or experience.
* Administrative proficiency for accurate data management and reporting.
* Capability and capacity for service management at a senior level in a complex healthcare environment, including staff management and change management.
* Relevant professional clinical, social work, AHP, or nursing qualification.


Desirable Criteria

* MSc/MA relevant to the post (or equivalent) – achieved or working towards.


Knowledge and Experience

* Previous managerial experience in a large and complex team/service/organisation.
* Previous experience working in a management role within Adult Services or physical healthcare services, including comprehensive understanding of discharge to assess principles.
* Previous clinical experience of discharge within health and/or community settings.
* Demonstrated success in delivering change and performance with and through management of teams.
* Ability to work within the scope of own role, delegating to others as appropriate to ensure best use of skills and time.
* Analytical ability to solve complex problems and develop practical solutions.
* Strategic, tactical and creative thinking, prioritising work programmes amid competing demands.
* Ability to manage and deliver to deadlines and within resources, with a strong sense of personal and team accountability and clear understanding of delegated authority boundaries.


Desirable Knowledge

* Breadth of management experience across different environments (NHS or non‑NHS).
* Knowledge of the NCL discharge pathways and processes.


Specific Skills

* Excellent interpersonal and communication skills, with a track record in writing complex papers, policies and presentations.
* Advanced keyboard skills and knowledge of a full range of IT software, including databases, Microsoft Office and MS Teams.
* Flexible and adaptable to meet deadlines, commitments and priorities.
* Intellectual flexibility to look beyond existing structures, ways of working, boundaries and organisations to produce more effective and innovative service delivery and partnerships.
* High level of work organisation, self‑motivation, drive for performance and improvement, and flexibility in approach and attitude.
