We work integrated with primary care, ASC, therapists, urgent & intermediate care services, alongside our specialist services informing our ageing well MDTs, with a focus on TEP and enhanced care planning. The role involves working in patients' homes and as part of the Care Home Service.

As a Community Sister/Charge Nurse, your role is vital in coordinating care and delegating tasks to your team.
You will be an autonomous practitioner, providing holistic, patient-centered care. You will champion patients, demonstrate motivated leadership, and influence and inspire others. The candidate must have the knowledge and confidence to have delicate conversations around advanced care planning, including end-of-life care and treatment escalation, and make clinical decisions regarding resuscitation based on the patient's presentation.

Training will be provided to build on your existing knowledge, experience, and skills. Continued learning and access to accredited courses are expected.
You should be competent in diagnosing and investigating, with sound knowledge and judgment in evidence-based wound management, pressure ulcer prevention, continence management, palliative, and end-of-life care.
A full driving license and the ability to travel across Plymouth are required.
All staff are expected to work across a 7-day service.

Job responsibilities
Provide planned and unplanned care to patients and carers in a defined area as part of a 24-hour District Nursing Service.
Respond to urgent referrals, triage and accept appropriate referrals, and refer to other agencies as needed.
Assess, diagnose, plan, organize, and review complex care packages, including case conferences.
The candidate must have the knowledge and confidence to have delicate conversations around advance care planning, end-of-life care, and resuscitation decisions.
Ensure privacy, dignity, and human rights are respected. Conduct comprehensive risk assessments to ensure safety.
Oversee equipment use, maintenance, and repair. Report incidents and near misses related to safety, security, and abuse.
Maintain accurate, confidential, and up-to-date records, adhering to policies and guidelines. Oversee record standards through audits and peer reviews.
Comply with infection control policies, attend updates, and escalate issues as necessary.
Monitor health, safety, and security, organize own time, and adapt to challenging environments like patients' homes.
Act as a clinical advisor within your expertise, promote health, and contribute to projects, research, and audits.
Work alongside the Community Matron for Long Term Conditions if appropriate, to monitor complex needs and reduce hospital admissions.
Prioritize and delegate tasks effectively, implement clinical supervision, and participate in annual appraisals and training.
Follow safeguarding procedures, support workload equity, and deputize for the District Nursing Manager as needed.
