We are looking to recruit - x hour temporary ( months)

The position involves sourcing care and support in response to an identified care plan, maintaining effective information about available services to support the care management and commissioning process, including communication with service users, families, and professionals. Negotiating cost within a competitive framework, liaising with families, service users and providers to progress the care and support delivery.

What we are looking for:
10. Experience of working in multi-agency and multidisciplinary setting and systems
11. Experience of working in a fast paced and pressured environment
12. An understanding of care coordination
13. Experience in using Liquid Logic and ContrOCC
14. The ability to co-ordinate support using a wide variety of care options direct payment, commissioned service, community sector organisation
15. Excellent written and verbal communication skills
16. A Strong team player across own and other teams internally and externally, actively contributing to the positive development of the team
17. Commitment to excellent customer service
