Role

Are you passionate about supporting children and young people with complex health needs? We are looking for a Fixed Term (12 months) 0.6 WTE dedicated Band 6 Children Community Nurse (CCN) to join our team, delivering preventative and supportive care for children and young people with complex health needs who live within the catchment of Knowsley. You will be working under the guidance of the CCN team manager and clinical lead and alongside a multidisciplinary team, inter agency team that play a vital role in prevention of GP and Hospital attendances for this population of children/ young people (CYP).

The role does require the successful applicant to be confident and competent in the management of clinical assessment and management of the acutely unwell CYP, and to understand the variance from the clinical norms that a CYP with complex health needs may present with. You will document these clinical variances using evidence-based practice to develop a care plan unique to the CYP with varying and challenging health needs.

Responsibilities

* Manage a defined caseload of children within community nursing, delivering specialist assessment, planning, implementation and evaluation, with the freedom to prioritise individual needs within Mersey Care policies.
* Be the accountable practitioner for the defined caseload, prioritising and delegating planned interventions to team members in line with individual skills and abilities.
* Be responsible for monitoring and evaluating work delegated from the defined caseload to team members.
* Ensure that organisational policies, Nursing and Midwifery Council (NMC) standards and competencies are adhered to by team members.
* Establish and maintain a good working relationship with primary and community care colleagues and other service providers, e.g., education, to provide a comprehensive service to children and young people.
* Agree and contribute to setting team objectives and delivering agreed outcomes.
* Support the aim to reduce hospital admissions, enable safe discharges, and help children and young people to live as fully and independently as possible within their communities with dignity, choice and inclusion at the centre of care.
* Work in collaboration with partner agencies (voluntary and statutory) to achieve good health outcomes for children and their families.
* Ensure delivery of the quality strategy within the designated area, including establishment of systems and processes.


Qualifications and Essential Criteria

* Confident and competent in clinical assessment and management of the acutely unwell CYP with complex health needs.
* Understanding of the variances from clinical norms in CYP with complex health needs and ability to document these variances.
* Experience in applying evidence-based practice to develop a care plan for CYP with varying and challenging health needs.
