Positions: Frailty Pharmacist
Location: St Helens South Primary Care Network, based at the Hub
Hours: 22.5 per week
Salary: Competitive, dependent on experience
Contract: Permanent
Main duties of the job
The role involves:
* Conducting structured medication reviews for frail or elderly housebound patients
* Tackling inappropriate polypharmacy and leading deprescribing initiatives
* Supporting end-of-life care planning and safe medicines management
* Working closely with GPs, frailty matrons, geriatricians, nurses, care coordinators, and social prescribers
* Providing medicines optimisation training and leading quality improvement projects
About You
We are seeking a pharmacist who is:
* Skilled in medicines optimisation and polypharmacy management
* A confident communicator who thrives in a multidisciplinary team
* Experienced in working with frail or elderly patients (desirable)
* IT-proficient, ideally with EMIS
* A car driver with business insurance (preferred for community-based visits)
* Willing to undergo an Enhanced DBS Check
* Having the right to work in the UK
