About The Role

We are at the forefront of community mental health transformation, operating from the dedicated Erith Centre within fully integrated, multi-disciplinary teams (MDTs). Our innovative approach, in partnership with Oxleas NHS Foundation Trust and Mind in Bexley, ensures holistic support that goes beyond medical intervention to address housing, employment, relationships, and finance – the things that truly matter to our service users.
You will work in one of our core community mental health pathways: the Integrated Community Mental Health Pathway (ICMP), the new Mental Health & Wellbeing HUB, and the ADAPT (Anxiety, Depression, Affective Disorders, Personality Disorders, and Trauma) Service.
The Role
This is an exciting opportunity for a motivated and skilled Social Worker to provide high-quality, recovery-focused social care services to adults experiencing complex mental ill-health. You will be a vital member of our MDTs, championing the social care perspective and ensuring the rights of our service users are upheld.
Key Responsibilities:
* Comprehensive Assessment & Planning: Conduct highly specialist social care assessments under the Care Act 2014, developing personalised, recovery-focused care plans in collaboration with service users and their carers.
* Integrated Working: Collaborate closely with multi-disciplinary colleagues (psychiatrists, nurses, OTs, etc.), primary care, and voluntary sector partners to ensure seamless, person-centred support.
* Complex Caseload Management: Hold a caseload of individuals with severe and enduring mental health needs (e.g., schizophrenia, bipolar disorder, personality disorders), providing interventions and support to maximise their independence and well-being.
* Safeguarding & Risk Management: Take a lead role in identifying and responding to safeguarding adults concerns, conducting risk assessments, and creating safety plans to protect vulnerable individuals.
* Advocacy & Resources: Act as an advocate for service users, ensuring access to appropriate local authority and community resources, including housing, benefits, and support networks.
* Statutory Duties: Lead on interpreting and applying relevant legislation, including the Care Act 2014, Mental Health Act, and Mental Capacity Act.
About You
We are looking for a passionate and resilient social worker who is committed to strength-based, systemic practice. You thrive in a dynamic, integrated environment and are dedicated to improving the lives of people with complex mental health needs.
Essential Requirements:
* A degree in Social Work.
* Current registration with Social Work England.
* Relevant experience working within a mental health setting/community mental health team.
* Strong knowledge of relevant legislation (Care Act, MHA, MCA).
* Excellent assessment, communication, and interpersonal skills.
* Ability to work autonomously and as part of a busy MDT, prioritising work effectively.
