Role Overview
Successful candidates will have experience in prioritising patients within ward areas, reducing missed doses by ensuring critical medications and complex patients are prioritised, and ensuring Medicines Reconciliation is completed within 48 hours. They will also provide discharge facilitation to patients highlighted for discharge, manage clinical queries and delegate tasks to the multidisciplinary team, and support the ward clinical team.
Key Responsibilities
* Prioritise patients within ward areas and critical medications
* Complete Medicines Reconciliation within 48 hours target times
* Provide discharge facilitation for patients
* Manage clinical queries and delegate appropriately
* Support dispensary and decentralised hubs, performing final checking duties and dispensing medicines
Qualifications & Experience
* Registered Pharmacy Technician with the GPhC
* Accredited Checking Pharmacy Technician qualification
* Medicines Reconciliation qualification
* Current or recent ward‑based experience
* Excellent communication and interpersonal skills, able to manage workload and motivate others
* Computer literacy; ability to work independently and as part of a team
