24-hour supported living / mental health & learning disability service Job Title: Learning Disability Support Worker (24-Hour Service) Location: London- Southeast Reports to: Team Leader / Service Manager Contract Type: Full-time / Part-time / Bank (5 x Staff Required), Salary: £12.21 - £13.50 Shifts: Rotational shifts including early, late, weekends, bank holidays, waking nights Job Purpose To provide high-quality, person-centred support to adults with learning disabilities and associated needs within a 24-hour supported living/residential service. The postholder will promote independence, dignity, choice and community inclusion while ensuring the safety, wellbeing and rights of the individuals we support at all times. Key Responsibilities Person-Centred Support Support individuals in accordance with their person-centred support plans and risk assessments. Promote independence with daily living skills such as cooking, shopping, budgeting, cleaning and managing appointments. Encourage individuals to make informed choices and participate in decisions affecting their lives. Support access to community activities, education, employment and social opportunities. Promote dignity, privacy, equality and respect at all times. Personal Care Provide personal care support including washing, dressing, toileting and continence care where required. Support mobility, safe moving and handling using appropriate equipment and training. Support with nutrition, meal preparation and dietary requirements. Administer medication safely following training and company policy. Health & Wellbeing Monitor physical and emotional wellbeing and report any concerns to senior staff. Support individuals to attend GP, hospital and specialist appointments. Follow healthcare plans (e.g., PEC feeding safety plans, epilepsy protocols, mental health support plans). Accurately record observations, incidents and health concerns. Behaviour Support Support individuals who may experience behaviours of distress using Positive Behaviour Support (PBS) approaches. Implement behaviour support plans and proactive strategies to reduce triggers. Use de-escalation techniques to maintain a calm and safe environment. Apply trained Breakaway techniques where necessary to ensure safety. Report and record incidents in line with policy. Safeguarding Safeguard vulnerable adults from abuse, neglect and exploitation. Recognise and report safeguarding concerns immediately in accordance with safeguarding procedures. Maintain professional boundaries and confidentiality. Record Keeping & Communication Complete daily notes, handovers, incident reports and body maps accurately. Communicate effectively with families, healthcare professionals and colleagues. Attend staff meetings, supervision and mandatory training. Household & Environmental Duties Maintain a clean, safe and homely environment. Support individuals with household tasks and meal preparation. Complete health and safety checks including fire checks and environmental checks. 24-Hour Service Responsibilities Work flexibly across day, evening, weekend and night shifts. Complete sleep-ins or waking night duties as required. Respond appropriately to emergencies and night-time support needs. Participate in shift handovers to ensure continuity of care. Person Specification Essential Genuine interest in supporting people with learning disabilities. Compassionate, patient and reliable approach. Good communication and teamwork skills. Ability to work flexible shifts including nights and weekends. Basic IT and written record-keeping skills. Understanding of dignity, equality, choice and inclusion. Desirable Previous experience in care/support work. NVQ/QCF Level 2 or 3 in Health & Social Care (or willingness to work towards). Knowledge or experience of Autism, ADHD, epilepsy or mental health needs. Knowledge of Positive Behaviour Support (PBS) and Breakaway techniques (advantage). Medication administration experience.
