Job Summary
Health and wellbeing coaches support people to take proactive steps to improve the way they manage their physical and mental health conditions, based on what matters to them. They help people develop their knowledge, skills and confidence in managing their health and care, improve their health outcomes and quality of life, and make positive changes in health‑related behaviours. Health coaches achieve this through coaching and motivation across multiple sessions, identifying needs, setting goals, and providing access to interventions such as self‑management education and peer support.
Main Duties
* Provide one‑to‑one and group health coaching support for people based on what is important to them.
* Manage and prioritise a caseload, in accordance with the needs and support required by individuals, and refer when the needs are beyond the scope of the health and wellbeing coach role.
* Work as part of a multidisciplinary, multi‑agency team to promote health coaching and serve as an ambassador for Personalised Care and Supported Self‑Management.
* Ensure that GPs, nurses, pharmacists and other members of the primary care team understand the health coaching role, how to refer, and which patients may benefit.
* Support local health, social care and voluntary sector professionals to make appropriate referrals to the service.
* Attend and contribute to team meetings within the practice.
* Adapt flexibly to the needs of the service and client group while maintaining the integrity of the role.
* Participate in regular health coaching supervision and continual learning, including regular contact with supervisors, refresher training sessions, peer support, e-learning, and ongoing improvements to systems and processes.
Key Tasks
* Meet people on a one‑to‑one or group consultation basis by phone, video conference or face‑to‑face.
* Build trust and respect, providing non‑judgemental and non‑discriminatory support while respecting diversity and lifestyle choices.
* Use a strength‑based approach, focusing on individuals’ assets.
* Apply a structured framework to coach individuals across a series of sessions: identify priorities, set personal goals, build skills and confidence, and use problem‑solving to overcome challenges.
* Apply the principles of self‑management, including shared decision‑making with healthcare professionals, engagement with personalised care plans, delivery of education and peer support, proactive engagement with social prescribing and community activities, and access to care‑coordinators or personal health budgets where needed.
Referrals & Promotion
* Promote health coaching in multidisciplinary meetings and give feedback on its impact.
* Develop strong links with local organisations to encourage referrals and provide them with regular updates.
* Raise awareness of coaching, especially to communities that experience barriers to access, and encourage equality, inclusion and case‑finding.
Miscellaneous
* Establish strong working relationships with GPs, practice teams and other health professionals, collaborating and supporting each other as a team.
* Act as a champion for health coaching as part of the practice’s personalised care offer.
* Carry out other duties as reasonably required within the general character of the post.
* Identify opportunities and gaps, review risks, and provide feedback to improve the service and contribute to business planning.
* Contribute to policies and plans relating to equality, diversity and health inequalities.
Qualifications & Experience
* Relevant Health and Wellbeing Coach qualification, including the ability to work with patients under 18 years old.

Salary: £31,049 to £37,796 a year (dependent on experience)
