Your Role
As a Community Health Nurse, you will play a vital role in enhancing the health and well-being of our community. Your focus will be on:
* Patient Care & Service: Conduct comprehensive assessments and provide skilled nursing care tailored to each client's needs.
* Planning: Develop individualized care plans in collaboration with our interdisciplinary health team.
* Communication: Maintain open lines of communication with patients, families, and staff to ensure seamless care.
* Documentation: Accurately document client care and implement physician orders in accordance with standards.
* Compliance: Adhere to all relevant regulations and educational requirements.

Qualifications
Education: Associate's Degree in Nursing (Required)
Licenses / Certifications:
* BLS - Basic Life Support - American Heart Association (AHA)
* RN - Registered Nurse - New York State Education Department (NYSED)
Physical Requirements
This role involves medium work, requiring the ability to exert 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently. Frequent walking, standing, or squatting is also necessary.
Compensation
Pay Range: $73,528.00 - $98,072.00
The listed base pay range is a good faith representation of current potential base pay for a successful full-time applicant. It may be modified in the future and is eligible for additional pay components based on experience, qualifications, and other factors.
