An exciting opportunity has arisen for a Clinical Pharmacist to develop a pioneering role within primary care. West Lancashire GP Federation is looking for a Clinical Pharmacist to join the Skelmersdale PCN.

The Clinical Pharmacist is a patient-facing role with opportunities to utilize clinical skills to improve community health. Responsibilities include managing long-term conditions, supporting hospital discharge prescribing, advising on multiple medications, and developing the pharmacist role within GP practices. The pharmacist will act as a specialist resource for medicines-related queries, signposting to other services, and liaising with healthcare professionals.

The role involves working independently and as part of a team to ensure safe and effective medicines use within the medicines optimisation agenda.

Job responsibilities

The role focuses on Primary Care Network initiatives, NHS Long Term Plan, and agreed deliverables to improve patient care. Key responsibilities include:

* Managing patients with chronic diseases and complex polypharmacy.
* Clinically assessing and treating patients with expertise in medicines.
* Providing specialist advice to address public health and social care needs and reduce inequalities.
* Collaborating with pharmacy and healthcare professionals across the sector.
* Leading person-centered medicines optimization and quality improvement initiatives.


Person Specification


Qualifications

* Accredited MPharm degree
* GPhC registration and successful registration assessment
* Clinical skills and diagnostics qualification


Experience

* Autonomous pharmacist practice
* Clinical management, service delivery, audit, research, health education
* Community and primary care experience, including case management


Skills and Knowledge

* Clinical leadership and management of long-term and complex conditions
* Knowledge of elderly patient needs, health policies, and clinical governance
* Experience with multidisciplinary teams and clinical examination skills
* Understanding of primary care changes and advanced clinical practice skills
