An exciting opportunity has arisen for a Clinical Pharmacist to develop a pioneering role within primary care. West Lancashire GP Federation is looking for a Clinical Pharmacist to join the Skelmersdale PCN.
The Clinical Pharmacist is a patient-facing role with opportunities to utilize clinical skills to improve community health. Responsibilities include managing long-term conditions, supporting hospital discharge prescribing, advising on multiple medications, and developing the pharmacist role within GP practices. The pharmacist will act as a specialist resource for medicines-related queries, signposting to other services, and liaising with healthcare professionals.

* Collaborating with pharmacy and healthcare professionals across the sector.
* Leading person-centered medicines optimization and quality improvement initiatives.
Person Specification
Qualifications
* Accredited MPharm degree
* GPhC registration and successful registration assessment
* Clinical skills and diagnostics qualification
Experience
* Autonomous pharmacist practice
* Clinical management, service delivery, audit, research, health education
* Community and primary care experience, including case management
Skills and Knowledge
* Clinical leadership and management of long-term and complex conditions
* Knowledge of elderly patient needs, health policies, and clinical governance
* Experience with multidisciplinary teams and clinical examination skills
* Understanding of primary care changes and advanced clinical practice skills
