Job Summary
Deal & Sandwich Primary Care Network (PCN) is seeking a motivated and experienced Paramedic to join our expanding community team. This community‑based role involves working across patients’ homes, care homes and community settings to deliver high‑quality, person‑centred care. You will support patients closer to home, assessing, treating and managing health needs while helping to reduce avoidable hospital admissions.
Main Duties
Undertake pre‑arranged and same‑day visits to patients in the community and care settings, providing comprehensive clinical assessment, diagnosis and treatment within professional scope. Conduct regular care‑home ward rounds under the Enhanced Health in Care Homes (EHCH) framework, completing timely reviews for new residents and hospital discharges, including personalised care planning and DNACPR/ReSPECT documentation when appropriate. Collaborate closely with GPs, nurses, pharmacists, social prescribers, care coordinators and wider PCN teams to deliver integrated, patient‑centred care. Apply clinical judgement to prioritise caseloads, initiate investigations and raise care when required. Promote preventative health, self‑care and patient education to improve long‑term outcomes and reduce unplanned admissions. Provide advice and clinical support to care‑home and community staff, maintaining accurate, contemporaneous clinical records on EMIS and other systems. Engage in continuing professional development, supervision, reflective practice, and contribute to service development, innovation and quality improvement initiatives across the PCN. Undertake any other duties agreed with line management to meet service needs.

Person Specification
Essential Qualifications
 * BSc in a training programme approved by the College of Paramedics
 * Active and relevant professional registration with the HCPC
 * Completed two‑year Consolidation of Learning period as a "newly qualified paramedic" and further three years of experience as a band 6 (or equivalent) paramedic
 * Commitment to obtaining the clinical pillar competencies of the paramedic FCP/AP roadmap within six months of commencement
Desirable Qualifications
 * Evidence of verification against the paramedic primary care roadmap

 * Understands and acts in line with HCPC professional standards for practice
 * Compassionate, adaptable, community‑focused approach
 * Commitment to providing safe, effective, person‑centred care
 * Ability to write comprehensive, accurate clinical notes and implement care plans
 * Good IT skills, including accurate clinical documentation

 * Ability to work under pressure and meet deadlines
Desirable Knowledge and Skills

 * Experience managing the care of patients presenting with undifferentiated undiagnosed conditions and/or designated caseloads, including examination, assessment, care planning, intervention and evaluation
 * Experience in urgent, emergency, or community‑based care
 * Strong clinical assessment and decision‑making skills
 * Experience working within a multi‑disciplinary team
 * Experience working independently in a patient‑focused environment
Desirable Experience
 * Experience in a primary care or care‑home environment
 * Evidence of working across organisational boundaries within health and social care
 * Knowledge of the EHCH framework
 * Experience with EMIS
