Job overview

If you are an enthusiastic, experienced Community Staff Nurse, this is an exciting opportunity to enhance and develop your leadership knowledge and skills within the dynamic role of a Community Nursing Deputy Team Leader.

This permanent position will see the successful candidate working alongside the District Nurse Team Leader, assisting in managing and leading a Community Nursing Team. In addition, you will work in collaboration with the wider Integrated Community Care/Primary Care services to ensure the delivery of safe, effective and patient focused planned and unplanned care at home.

Main duties of the job

In collaboration with the District Nurse Team Leader, lead and manage the caseload of the Community Nursing Team, monitoring standards of care, delegating responsibilities and supervise team members as appropriate.

Develop and maintain appropriate communication systems. Working closely with patients/carers and families and other members of the integrated care team and statutory and voluntary agencies.

Working in partnership with other agencies, act as care manager for identified patients in order to prevent inappropriate hospital admission.

In collaboration with the wider integrated care team provide care planning, support, treatment and monitoring to patients on the general case load.

In collaboration with the Team Leader, manage resources effectively and advise on skill mix requirements for the team.

In collaboration with the Team Leader, assist with the management of team budgets.

Positively participate in and promote Clinical Governance, thus ensuring the highest quality of practice is maintained, within the service. Demonstrate the use of evidence based practice and participation in clinical audit.

In collaboration with the Team Leader assist with performance reviews within the team in order to contribute to team and corporate objectives, encouraging and supporting development of staff and maintenance of personal development plans.

Ensure team members undertake mandatory training requirements.

Qualifications

Essential criteria

1. Registered Nurse Level 1
2. District Nurse degree or equivalent. (or a willingness to complete DN Qualification as directed by management)
3. Experience of administration of Intravenous medications (or a willingness to complete training within an agreed timescale)
4. To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB998.

Desirable criteria

5. Community Practitioner Nurse Prescriber / Independent Prescriber
6. Masters level study in long term conditions
7. Mentorship qualification or equivalent

Experience

Essential criteria

8. Experience of working with; Older people, Palliative Care, chronic disease
9. Continuing professional knowledge and development.
10. Experience of teaching and mentorship.
11. Evidence of knowledge and experience of Case Management
12. Change management and leadership skills
13. Working in partnership with other healthcare providers, Social services etc
14. Experience In the management of staff
15. Experience of multidisciplinary working

Desirable criteria

16. Mental Health Experience
17. Experience of multidisciplinary team working and Case Management models

Skills and Knowledge

Essential criteria

18. Sound understanding of Primary care and multi agency working
19. Awareness of the value of audit/research and evidence based practice
20. Awareness of NHS issues

23. Evidence of ability to communicate both verbally and in writing at all levels in multi-disciplinary forum and with users and carers.
24. Able to organise and coordinate services in collaboration with, and in the absence of the District Nurse Team Leader

28. Due to the requirement to undertake visits and transport non-portable equipment, the post holder is required to be either a car driver with a valid driving licence, or have access to an alternative means of transport.
