Note: The Haematology department is looking for an enthusiastic, forward-thinking Care Support Worker to join the team. The Care Support Worker will work with the Haematology CNS team to provide care to Haematology patients and will coordinate care for people with non-complex care needs. This includes joining up services, coordination, information and communication between carers, treatment providers and patients living with and beyond cancer and their families to create a seamless experience of care.
Responsibilities
* Coordinate care by providing a single point of access, including rapid re-entry into the system for people identified as having urgent or specialist needs; provide general information and support about cancer and cancer services; enable patients to navigate the health and social care system and make choices that are best for their cancer and their life.
* Triage incoming calls and initiate appropriate responses according to protocols and individual pathways; use good communication skills, basic clinical awareness and appropriate tools and procedures; liaise with the team for non-routine or complex decisions.
* Coordinate care for a defined group of patients assessed as having level one care needs for supported self-management.
* Under the guidance and supervision of a registered practitioner, be a member of an existing team and coordinate care with a focus on vulnerable people; ensure information is shared between care givers, treatment providers and patients and families.
* Work with macrophin cancer support activities as part of Macmillan care services and participate in related training and development where required.
Qualifications
* NVQ4 or NVQ3 plus additional training to diploma level equivalent
* GCSE English Language and Mathematics (Grades A-C) or equivalent
* Essential criteria: experience of working with multiprofessional team; evidence of good communications skills and use of data management
* Desirable criteria: evidence of continued role development; willingness to undertake Macmillan learning and development courses; understanding of Macmillan Cancer Support and its role across the UK; experience in coordinating a patient workload
* Disclosures: The postholder will have access to vulnerable people; Disclosure to DBS is required in line with policy
