Band 6 District Nurse / Case Manager – Alsager (Secondment – 7 months)
An exciting opportunity has arisen to work as a Band 6 District Nurse / Case Manager in Alsager within the SMASH Care Community Team as a secondment post for 7 months.
You will be joining a caring, compassionate and hardworking team who work together to provide the best care for patients in the area and who pull together to look after each other, working to the value that everyone matters.
In this role you will work in partnership with patients and their families to lead the delivery of person‑centred, safe and effective care. You will demonstrate sound clinical knowledge and skills in the management of patients with complex needs, long‑term conditions and end‑of‑life care. You will be responsible for managing and coordinating patients requiring community nursing input, overseeing the assessment of complex nursing care needs, and developing, implementing and evaluating programmes of individualised care for patients on your defined case load.
You will deputise for the team leader as required, ensuring continued efficient and effective service delivery. Working as part of the integrated community health care team, you will cover the 24‑hour period of service delivery across professional and organisational boundaries. You will support the district nurse team leader in providing clinical leadership to the community nursing team and assist in leading the team in the provision of high‑quality, evidence‑based programmes of care.
Qualifications and experience: Extensive clinical experience and case management experience, preferably within the community setting, and recent and ongoing clinical professional development relevant to this post. You will have strong leadership, organisational and communication skills and be able to manage challenging situations in a professional and flexible manner. Travel is an essential part of this role; please demonstrate how you would meet this requirement.
