Job overview

The adult learning disabilities Health Support Team is looking for a full time Band 3 health support worker to work in our community multi disciplinary team in Bradford. The post holder needs to have an understanding of the health needs of adults with learning disabilities across the Bradford district, and be able to support the team to help reduce the health inequalities our patient population faces. This may be around their physical health or their emotional wellbeing and is a hugely varied role. An excellent understanding of reasonable adjustments and how to make these is required.

The post holder will be based specifically in our health facilitation learning disabilities nursing team at Waddiloves, working alongside the nurses to support people to access appropriate healthcare. However you also need to be able to travel around the district as we cover a large area.

The post at times involves lone working, visiting people in their own home, at their day time activities, or within other local services. The post holder will also work alongside other members of the MDT and needs to be an excellent communicator and enjoy being part of a friendly, supportive team. The ability to be able to manage their own time, prioritise workload and stay calm are all key skills as is having a friendly and approachable manner. Being able to speak languages other than English would also be beneficial and the person needs to have access to a car and be able to drive as we cover a large patch from central Bradford out to Skipton and beyond.

Main duties of the job

To work as part of the Learning Disabilities Health Support Team. Ensuring high quality health care is delivered to
adults with learning disabilities. To work in collaboration with the wider learning disabilities multi-disciplinary team and local health
providers, whilst participating in the assessment of health needs and delivering of care.

The post holder is expected to work as a member of the Learning Disabilities Health Support Team and assist the health facilitation team
nurses, matrons and wider MDT as directed. The post holder will be responsible for delivering programmes of care under the direct or indirect supervision of a qualified nurse, whilst fostering both the team's and organisation's philosophy and approach to care. The post holder will undertake clinical activities that are identified within the aligned role specific skills matrix.
