We're recruiting 2x Mental Health Practitioners specialising in Adult mental health to join the Gateway Team in Aylesbury!
We are looking for autonomous, driven, and confident practitioner(s). You will have the opportunity for continuing professional development (CPD), with scope to influence service innovation and development. You will also provide high-quality comprehensive telephone assessments for those identified as needing mental health support within a secondary mental health setting.

Main duties of the job
* Serve as the main point of contact within the Gateway, making clinical decisions regarding referrals and providing advice and support to professionals, service users, and families
* Perform comprehensive triage functions in line with the Trusted Assessor Model, including risk assessment and evaluation of new referrals
* Provide specialist advice to referrers and liaise with service users to establish their current presentation, offer advice, and formulate management and safety plans while they await specialist assessment
* Support the delivery of high-quality link worker provision into primary care, secondary mental health services, social care partners, community-based resources, and the voluntary sector, including providing consultation and training to staff in various agencies
* Assist the Gateway/Team Operational Lead in delivering the service, ensuring it meets national guidance and local key performance indicators, and support the learning and development of new team members

Gateway Mental Health Practitioners are being recruited from a range of disciplines registered with a professional body; Occupational Therapists, Mental Health Social Workers, and Mental Health Nurses. all roles are vital to implementing specialist mental health triage functionality within Buckinghamshire.
